During childbirth, the baby comes out of the uterus, down through the vaginal canal, and out through the vaginal opening head first.  Now, a baby’s head is very hard and unforgiving, and it’s usually about 10cm around at the largest point.  As it comes out of the vaginal opening it stretches skin and tissue and muscle that is usually able to stretch as much as necessary.  However, about 1/3 of the time, the baby comes out too fast or there’s a weak spot in the tissue or the baby has a hand up or some other reason, and the tissue tears at that point.
